Ramadan Arts & Crafts Workshop for Children

I, the parent/guardian of the child listed, agree to the following terms:

1. Parental Accompaniment and Responsibility: I understand that a female parent/adult must accompany my child/children throughout the workshop. I acknowledge full responsibility for the health, safety, and well-being of my child/children during the entire duration of the workshop.
2. Medical Consent: I authorise the workshop staff to seek emergency medical treatment for my child if deemed necessary.
3. Liability Waiver: I acknowledge that participation in workshop activities involves inherent risks. I agree to release and hold harmless Easton Islami Darasgah (Greenbank Masjid) and its staff from any claims arising from my child’s participation in the workshop, except in cases of gross negligence.
4. Behaviour Agreement: I understand that my child is expected to:
   Behave appropriately
   Follow workshop rules and instructions
   Respect other participants and staff I acknowledge that the workshop reserves the right to remove my child from activities if their behaviour is disruptive or poses a safety risk.
5. Health Declaration:
   I confirm that the health information provided for my child is accurate and complete
   I agree to notify workshop staff of any changes to my child’s health status prior to the event
   I will inform staff of any allergies, medical conditions, or special needs
6. Personal Items: I understand that Easton Islami Darasgah (Greenbank Masjid) is not responsible for lost, stolen, or damaged personal items brought to the workshop.
